Purpose | (Clinical Focus: The intent of this value set is to represent SBD and BPCK moderate intensity statin medications as defined by the 2018 AHA/ACC/AACVPR/AAPA/ABC/ACPM/ADA/AGS/APhA/ASPC/NLA/PCNA Guideline on the Management of Blood Cholesterol.),(Data Element Scope: RxNorm SBD and BPCK moderate intensity statin medications as defined by the 2018 AHA/ACC/AACVPR/AAPA/ABC/ACPM/ADA/AGS/APhA/ASPC/NLA/PCNA Guideline on the Management of Blood Cholesterol. NOTE: The medications in this value set do not align with the moderate intensity statins present in 2.16.840.1.113762.1.4.1047.98, since that value set represents the 2013 ACC/AHA listing of moderate intensity medications.),(Inclusion Criteria: Semantic branded drugs (SBD) and semantic branded drug packs (BPCK) codes that represent moderate intensity statin medications defined by the 2018 AHA/ACC/AACVPR/AAPA/ABC/ACPM/ADA/AGS/APhA/ASPC/NLA/PCNA Guideline on the Management of Blood Cholesterol. NOTE #1: Per the 2018 guidelines, Lovostatin 40-80 mg is now considered a moderate intensity statin (versus Lovostatin 40 mg in the 2013 guideline) AND Pitavastatin 1-4 mg is now considered a moderate intensity statin (versus Pitavastatin 2-4 mg in the 2013 guidelines). NOTE #2: Liquid statin medications are included in this value set (e.g., Simvastatin 4 mg/ml and Simvastatin 8 mg/ml). Be aware that the quantity of milliliters or tablets are required to confirm that the patient is taking the equivalent of a moderate intensity statin versus a low intensity statin.),(Exclusion Criteria: Semantic clinical drugs (SCD) and semantic clinical drug packs (GPCK) RxNorm codes that represent moderate intensity statin medications, along with any RxNorm codes that represent high or low intensity statin medications.) |
